1. The Amref Medical Centre is reliant on the ministry of health, to make the vaccine against COVID-19, available. We cannot guarantee when and which vaccination will be available when we contact you with regards to vaccine availability.

2. As a potential vaccine recipient, you are responsible for ensuring you inform the medical staff about any known allergies or underlying conditions which may make you vulnerable to vaccine side effects or which may make you ineligible for the COVID-19 vaccination.

3. Where a second dose of the vaccination against COVID-19 is required, you will be required to provide proof of initial vaccination.

4. By registering, you agree to retain your individual responsibility, to seek vaccination at the earliest opportunity provided to you, even if that opportunity is not presented at Amref Medical Centre.

5. Whereas the Clinical team is well trained and certified in the management of medical emergencies including drug reactions, the organization will not be liable for any eventuality associated with the administration of the vaccination against COVID-19.

6. Any adverse reaction following vaccination, should be reported using Ministry of Health approved channels https://pv.pharmacyboardkenya.org
